[Español] Objetivos: Analizar la terapia fotodinámica como coadyuvante en el tratamiento periodontal no quirúrgico a partir de la evidencia científica disponible. Se buscó describir los mecanismos de acción antimicrobiana e inmunomoduladora, evaluar la efectividad clínica como complemento del raspado y alisado radicular, y analizar las principales ventajas, limitaciones y consideraciones clínicas de su aplicación en periodoncia. Metodología: Se realizó una revisión descriptiva y analítica de la literatura científica publicada entre 2020 y 2025. La búsqueda se efectuó en bases de datos especializadas como EBSCO, PubMed, Lilacs y Google Scholar, así como en páginas web como Medscape y BMC, utilizando combinaciones de palabras clave con operadores booleanos. Se incluyeron artículos de investigación originales, revisiones sistemáticas y metaanálisis, excluyendo reportes de casos clínicos. Conclusiones: La terapia fotodinámica combina fotosensibilizadores con luz específica para generar especies reactivas de oxígeno que destruyen selectivamente bacterias periodontopatógenas y modulan la respuesta inflamatoria, promoviendo la regeneración tisular. La evidencia demuestra que su combinación con raspado y alisado radicular produce mejoras significativas en ganancia de inserción clínica, reducción de profundidad de sondaje y control del sangrado gingival, con resultados equivalentes a antibióticos sistémicos sin generar resistencia bacteriana. Representa una alternativa mínimamente invasiva especialmente útil en inflamación persistente, bolsas residuales o pacientes con contraindicación antibiótica. Sin embargo, la falta de estandarización protocolaria y la escasez de estudios longitudinales limitan su reproducibilidad clínica y aplicación universal.
[English] Objectives: To analyze photodynamic therapy as an adjuvant in non-surgical periodontal treatment based on available scientific evidence. The aim was to describe antimicrobial and immunomodulatory mechanisms of action, evaluate clinical effectiveness as complement to scaling and root planing, and analyze main advantages, limitations and clinical considerations of its application in periodontics. Methodology: A descriptive and analytical review of scientific literature published between 2020 and 2025 was conducted. The search was performed in specialized databases such as EBSCO, PubMed, Lilacs and Google Scholar, as well as websites like Medscape and BMC, using keyword combinations with Boolean operators. Original research articles, systematic reviews and meta-analyses were included, excluding clinical case reports. Conclusions: Photodynamic therapy combines photosensitizers with specific light to generate reactive oxygen species that selectively destroy periodontal pathogenic bacteria and modulate inflammatory response, promoting tissue regeneration. Evidence demonstrates that its combination with scaling and root planing produces significant improvements in clinical attachment gain, probing depth reduction and gingival bleeding control, with results equivalent to systemic antibiotics without generating bacterial resistance. It represents a minimally invasive alternative especially useful in persistent inflammation, residual pockets or patients with antibiotic contraindication. However, lack of protocol standardization and scarcity of longitudinal studies limit its clinical reproducibility and universal application.
