[Español] Introducción: La facoemulsificación es la técnica quirúrgica de referencia para el tratamiento de la catarata. La energía disipada acumulada (CDE) cuantifica la energía ultrasónica total entregada al ojo durante el procedimiento y constituye un marcador de eficiencia quirúrgica y predictor de daño endotelial corneal. Objetivo: Comparar el nivel de energía disipada acumulada entre las técnicas de Phaco Chop y Stop and Chop en la facoemulsificación de cataratas. Método: Se realizó un estudio observacional, analítico, comparativo y retrospectivo. Se analizaron 77 ojos de 40 pacientes intervenidos, mediante las técnicas Phaco Chop (n=45 ojos) y Stop and Chop (n=32 ojos) por un único cirujano experimentado, en el Departamento de Oftalmología del Hospital Escuela Dr. Jorge Abraham Hazoury Bahles (INDEN), durante el período junio-diciembre del año 2025. Se aplicaron las pruebas de Shapiro-Wilk, U de Mann-Whitney y correlación de Spearman (p < 0.05). Resultados: La muestra presentó un predominio del sexo femenino (55.00%) y una edad media de 60.2 ± 11.0 años. El grado de catarata más frecuente fue el N2 (41.6%). El CDE medio fue de 3.04 ± 1.97 %-seg para el Phaco Chop y de 6.00 ± 3.57 %-seg para el Stop and Chop, con diferencia estadísticamente significativa (U=327.50, p=0.0001). Se confirmó correlación positiva fuerte entre grado de catarata y CDE (rho=0.708, p<0.0001). Conclusión: La técnica Phaco Chop generó significativamente menor CDE que el Stop and Chop, con una ventaja energética que se acentúa a medida que aumenta la densidad nuclear del cristalino. Estos hallazgos respaldan la selección del Phaco Chop como técnica de elección en cataratas de grado moderado a avanzado para minimizar el daño endotelial corneal.
[English] Introduction: Phacoemulsification is the gold-standard surgical technique for the treatment of cataracts. Cumulative Dissipated Energy (CDE) quantifies the total ultrasound energy delivered to the eye during the procedure and serves as a marker of surgical efficiency and a predictor of corneal endothelial damage. Objective: To compare the level of Cumulative Dissipated Energy between the Phaco Chop and Stop-and-Chop techniques in cataract phacoemulsification. Methods: An observational, analytical, comparative, and retrospective study was conducted. A total of 77 eyes from 40 patients who underwent cataract surgery using either the Phaco Chop (n = 45 eyes) or Stop-and-Chop (n = 32 eyes) technique were analyzed. All procedures were performed by a single experienced surgeon at the Department of Ophthalmology of Dr. Jorge Abraham Hazoury Bahles Teaching Hospital (INDEN) between June and December 2025. Statistical analyses included the Shapiro–Wilk test, MannWhitney U test, and Spearman correlation test, with a significance level set at p < 0.05. Results: The sample showed a predominance of female patients (55.00%) with a mean age of 60.2 ± 11.0 years. The most frequent cataract grade was N2 (41.6%). Mean CDE was 3.04 ± 1.97 %-seconds for the Phaco Chop group and 6.00 ± 3.57 %-seconds for the Stopand-Chop group, demonstrating a statistically significant difference (U = 327.50, p = 0.0001). A strong positive correlation was confirmed between cataract grade and CDE (rho = 0.708, p < 0.0001). Conclusion: The Phaco Chop technique generated significantly lower CDE than the Stopand-Chop technique, with an energy-saving advantage that became more pronounced as lens nuclear density increased. These findings support the selection of Phaco Chop as the preferred technique for moderate to advanced cataracts in order to minimize corneal endothelial damage.
