| dc.description.abstract | [Español] Introducción: La enfermedad cerebrovascular (ECV) constituye una de las principales causas de morbimortalidad y discapacidad en el adulto mayor. Entre sus complicaciones más frecuentes se encuentra la disfagia orofaríngea, que compromete la seguridad y eficacia de la deglución y se vincula al desarrollo de malnutrición y neumonía por aspiración. El objetivo de este estudio fue determinar la frecuencia de disfagia post-ECV y su relación con el riesgo de malnutrición en pacientes geriátricos ingresados en el Hospital General Dr. Vinicio Calventi durante el período julio 2025 – enero 2026. Metodología: Se realizó un estudio observacional, analítico, de corte transversal, con recolección retrospectiva de datos a partir de la revisión de expedientes clínicos. De un total de 431 pacientes ingresados por el servicio de Geriatría durante el período de estudio, se incluyeron los 177 pacientes con diagnóstico de ECV que cumplieron los criterios de inclusión. La función deglutoria se evaluó mediante el Gugging Swallowing Screen (GUSS) y el riesgo nutricional mediante el Controlling Nutritional Status (CONUT). El análisis estadístico incluyó pruebas de chi-cuadrado y regresión logística, considerando significativo un valor de p < 0.05. Resultados: La frecuencia de disfagia fue del 83.6%, predominando las formas leve (41.2%) y moderada (32.2%). El 76.3% de los pacientes presentó algún grado de riesgo nutricional según el CONUT. No se halló asociación estadísticamente significativa entre la presencia de disfagia y el riesgo de malnutrición (χ² = 0.087; p = 0.768). Sin embargo, la disfagia se asoció de manera significativa e independiente con el requerimiento de sonda de alimentación (p < 0.001) y con la neumonía por aspiración (OR ajustado = 9.17; IC 95%: 2.94–28.60; p < 0.001). Conclusión: La disfagia y el riesgo nutricional son condiciones altamente prevalentes en el paciente geriátrico con ECV. Aunque no se demostró asociación directa entre ambas, la disfagia incrementó significativamente el riesgo de complicaciones de alto impacto clínico, lo que respalda la necesidad de implementar un tamizaje deglutorio y nutricional sistemático al ingreso. |
| dc.description.abstract | [English] Introduction: Cerebrovascular disease (CVD) is one of the leading causes of morbidity, mortality, and disability in older adults. Among its most frequent complications is oropharyngeal dysphagia, which compromises the safety and efficacy of swallowing and is linked to the development of malnutrition and aspiration pneumonia. The aim of this study was to determine the frequency of post-stroke dysphagia and its relationship with the risk of malnutrition in geriatric patients admitted to Hospital General Dr. Vinicio Calventi during the period July 2025 – January 2026. Methods: An observational, analytical, cross-sectional study was conducted, with retrospective data collection based on the review of clinical records. Out of a total of 431 patients admitted to the Geriatrics service during the study period, the 177 patients diagnosed with CVD who met the inclusion criteria were included. Swallowing function was assessed using the Gugging Swallowing Screen (GUSS) and nutritional risk using the Controlling Nutritional Status (CONUT) tool. Statistical analysis included chi-square tests and logistic regression, with a p-value < 0.05 considered significant. Results: The frequency of dysphagia was 83.6%, with mild (41.2%) and moderate (32.2%) forms predominating. A total of 76.3% of patients presented some degree of nutritional risk according to the CONUT score. No statistically significant association was found between the presence of dysphagia and the risk of malnutrition (χ² = 0.087; p = 0.768). However, dysphagia was significantly and independently associated with the need for tube feeding (p < 0.001) and with aspiration pneumonia (adjusted OR = 9.17; 95% CI: 2.94–28.60; p < 0.001). Conclusion: Dysphagia and nutritional risk are highly prevalent conditions in geriatric patients with CVD. Although no direct association between the two was demonstrated, dysphagia significantly increased the risk of clinically relevant complications, supporting the need to implement systematic swallowing and nutritional screening at admission. |
