[Español] Introducción: La sensibilidad corneal es un indicador funcional de la integridad de la inervación corneal que puede deteriorarse en el contexto de la neuropatía diabética. En la República Dominicana, su valoración sistemática en pacientes con diabetes mellitus tipo 2 (DM2) no se había integrado a la práctica clínica de rutina, pese a la alta carga de la enfermedad en el país. Métodos: Se realizó un estudio observacional, descriptivo-analítico y transversal en 113 pacientes con DM2 atendidos en el Hospital Escuela Dr. Jorge Abraham Hazoury Bahlés de Santo Domingo, durante el período marzo–mayo de 2026. La sensibilidad corneal fue valorada mediante estimulación mecánica con algodón y categorizada en cuatro niveles ordinales (normal, leve, moderada, severa). Su asociación con variables clínicas y metabólicas fue explorada mediante pruebas no paramétricas y regresión logística ordinal. Resultados: El 60% de los pacientes presentó algún grado de alteración corneal (leve 35%, moderada 16%, severa 8.8%). El tiempo de evolución de la DM2 mostró correlación positiva moderada con la severidad corneal (ρ = 0.381; p < 0.001). En el modelo multivariado, la neuropatía periférica diabética fue el predictor de mayor magnitud (OR = 3.02; IC 95%: 1.32–6.90), seguida de la hipertensión arterial (OR = 2.47; IC 95%: 1.13–5.40) y el tiempo de evolución (OR = 1.10; IC 95%: 1.04–1.16). No se identificaron asociaciones significativas con HbA1c ni glucemia reciente. Discusión: La alta prevalencia de alteración corneal y su asociación con predictores clínicos accesibles respaldaron la incorporación de esta evaluación en el seguimiento oftalmológico rutinario del paciente diabético en entornos con recursos limitados.
[English] Introduction: Corneal sensitivity is a functional indicator of corneal innervation integrity that may become impaired in the setting of diabetic neuropathy. In the Dominican Republic, its systematic assessment in patients with type 2 diabetes mellitus (T2DM) had not been integrated into routine clinical practice, despite the high disease burden in the country. Methods: An observational, descriptive-analytical, cross-sectional study was conducted in 113 patients with T2DM seen at the Dr. Jorge Abraham Hazoury Bahlés Teaching Hospital in Santo Domingo, from March to May 2026. Corneal sensitivity was assessed using mechanical cotton stimulation and categorized into four ordinal levels (normal, mild, moderate, severe). Its association with clinical and metabolic variables was explored using non-parametric tests and ordinal logistic regression. Results: Sixty percent of patients exhibited some degree of corneal impairment (mild 35%, moderate 16%, severe 8.8%). Diabetes duration showed a moderate positive correlation with corneal severity (ρ = 0.381; p < 0.001). In the multivariable model, diabetic peripheral neuropathy was the strongest predictor (OR = 3.02; 95% CI: 1.32–6.90), followed by arterial hypertension (OR = 2.47; 95% CI: 1.13–5.40) and diabetes duration (OR = 1.10; 95% CI: 1.04–1.16). No significant associations were found with HbA1c or recent blood glucose. Discussion: The high prevalence of corneal impairment and its association with accessible clinical predictors supported the incorporation of this assessment into routine ophthalmological follow-up of diabetic patients in resource-limited settings.
