[Español] La apnea obstructiva del sueño (AOS) es un trastorno común asociado con una morbilidad y mortalidad importante. Esta La cual tiene un efecto perjudicial significativo en la calidad de vida del paciente cuando no es detectada a tiempo. Entre los síntomas principales son: fuertes ronquidos, molestos, ahogos, jadeos, dificultad de mantener el sueño, trastornos hipertensivos y taquicardias. Objetivos: evaluar la eficacia a largo plazo del avance maxilomandibular (AMM) como tratamiento de la apnea obstructiva del sueño, por otra parte, determinar contraindicaciones, cambios de vías respiratorias, cambios del índice de apnea hipopnea y calidad de vida. Métodos: se realizó una revisión de la literatura científica entre los años 2015 al 2022, a través de los buscadores de información y plataformas JAMA, Medscape, ScienceDirect, Google Scholar, Pubmed, Medline entre otros, con los idiomas inglés y español. Discusión: estudio por Stefanovic determinó que la cirugía conllevó a un aumento significativo del volumen aéreo, Miranda y colaboradores definieron como bien tratamiento quirúrgico para la apnea obstructiva. Holty et al determinaron el éxito del procedimiento a través de la medida del IAH con una progresión del 85%. Conclusión: el AMM causa un aumento de las vías respiratorias superiores, los pacientes de mayor impacto son aquellos con obstrucción clase III, así como los que no les es efectivo el uso de CPAP y con cirugías anteriores no efectivas. De igual manera ocasiona cambios positivos a largo plazo, manteniendo una buena calidad de vida y mejor calidad del sueño.
[English] Obstructive sleep apnea (OSA) is a common disorder associated with significant morbidity and mortality. This which has a significant detrimental effect on the quality of life of the patient when it is not detected in time. Among the main symptoms are loud, annoying snoring, choking, gasping, difficulty staying asleep, hypertensive disorders and tachycardia. Objectives: to evaluate the long-term efficacy of maxillomandibular advancement (MMA) as a treatment for obstructive sleep apnea, on the other hand, to determine contraindications, airway changes, changes in the apnea-hypnea index and quality of life. Methods: a review of the scientific literature was carried out between 2015 and 2022, through the information search engines and platforms JAMA, Medscape, ScienceDirect, Google Scholar, Pubmed, Medline, among others, with English and Spanish languages. Discussion: a study by Stefanovic determined that surgery led to a significant increase in air volume, Miranda et al. defined surgical treatment for obstructive apnea as good. Holty et al determined the success of the procedure by measuring the AHI with a progression of 85%. Conclusion: MMA causes an increase in the upper respiratory tract, the patients with the greatest impact are those with class III obstruction, as well as those for whom the use of CPAP is not effective and with previous ineffective surgeries. In the same way, it causes positive changes in the long term, maintaining a good quality of life and better quality of sleep.
