[Español] Introducción: La introducción de la tomografía de coherencia óptica angiográfica AngioOCTA actualmente como un estudio diagnóstico no invasivo en anomalías retinianas, abrió expectativas en la investigación de la microcirculación, sobre todo en macula que es difícil detectarlo en fondo de ojo, esos signos clínicos no evidentes en pacientes diabéticos tipo 1 y 2 describiendo características en las primeras etapas de la retinopatía diabética, pero aún no hay consenso de su valor pronóstico. Encontramos que hay pocos estudios que evalúen todos los parámetros del AngioOCTA y que se relacionen con la agudeza visual, permitiéndonos determinar el área de la zona avascular foveal que forma parte de macula en pacientes con y sin datos de retinopatía diabética. Resultados: Se evaluaron 62 pacientes diabéticos tipo 1 y 2 con y sin datos de retinopatía diabética no proliferativa leve, moderada y severa sin edema macular para un total de 123 ojos, el sexo predominante fue el femenino con el 54.1% del total, la edad más frecuente es mayor de 60 años ,el tiempo de evolución de la diabetes mellitus más frecuente es de 15 años , siendo en pacientes diabéticos tipo 2, estos pacientes presentaron agudeza visual 20/30 para ojo derecho 24.6% y para ojo izquierdo 26.2%, el área de la zona avascular foveal más frecuente 0.04- 0.10mm² con un 49.2 % para ojo derecho y un 47.5% para ojo izquierdo, en pacientes con no datos de retinopatía diabética, el 32.8 % desconoce su estado metabólico HbA1c. Discusión: Se evidencia un aumento en el área de la zona avascular foveal en los pacientes diabeticos tipo 1 y 2. mostrando un vínculo entre la neurodegeneración retiniana y el deterioro microvascular de la retinopatía diabética no proliferativa y en las etapas preclínicas, sin ninguna disminución significativa para la función visual.
[English] Introduction: The introduction of Angio OCT Aangiographic optical coherence tomography, currently as a non-invasive diagnostic study in retinal abnormalities, opened expectations in the investigation of microcirculation, especially in the macula, which is difficult to detect in the fundus, these clinical signs not evident in type 1 and 2 diabetic patients describing characteristics in the early stages of diabetic retinopathy, but there is still no consensus on its prognostic value. We found that there are few studies that evaluate all the Angio OCT A parameters and that are related to visual acuity, allowing us to determine the area of the foveal avascular zone that forms part of the macula in patients with and without data of diabetic retinopathy. Results: 62 type 1 and 2 diabetic patients with and without data of mild, moderate and severe non-proliferative diabetic retinopathy without macular edema were evaluated for a total of 123 eyes, the predominant sex was female with 54.1% of the total, the most frequent age is older than 60 years, the time of evolution of the most frequent diabetes mellitus is 15 years, being in type 2 diabetic patients, they presented visual acuity 20/30 for the right eye 24.6% and for the lef teye 26.2%, the area of the most frequent foveal avascular zone 0.04-0.10mm² with 49.2% for the right eye and 47.5% for the left eye, they occur in patients with no data of diabetic retinopathy, 32.8% do not know their HbA1c metabolic status. Discussion: Evidence an increase in the area of the foveal avascular zone in type 1 and 2 diabetic patients and showing a link between retinal neurodegeneration and microvascular impairment of non-proliferative diabetic retinopathy and in the preclinical stages, without any significant decrease for visual function.
